The educational institution where you undertook / are undertaking your ROTC training should provide you with your serial number. If, for some reason, your educational institution is unable to provide you with the serial number, you may directly inquire with the Armed Forces of the Philippines (AFP) Office of the Deputy Chief of Staff for Reservist and Retiree Affairs (OJ9).
