• If you are a pensioner, go to the Veterans Record Management Division (VRMD) of the Philippine Veterans Affairs Office (PVAO) or the PVAO Field Service Extension Offices, and present the following requirements:

• Request from from VRMD
• Photocopy of two (2) valid ID's.
• Authorization letter, if the record-holder will be represented by another person

 

• If you are not a pensioner:

• Please proceed to the Non-Current Records Division, Office of the Adjutant General (NRD-OTAG) at the Armed Forces of the Philippines Headquarters; or
• Please prepare a written request to the U.S. Department of Veterans Affairs, Roxas Blvd, Pasay City; or
• Prepare a written request to the National Personnel Records Center (NPRC), 1 Archives Dr, St. Louis, Missouri 63138 U.S.A.

 

• The Philippine Veterans Affairs Office (PVAO) evaluates and processes non-pension claims for veterans and their qualified dependents. For the processing of educational benefits, billing and availment of the Veterans Hospitalization Program (VHP), PVAO may be reached through the following:
